What are Uterine Polyps?
Endometrial polyps are tiny tissue growths that originate from the uterine lining, or endometrium. Polyps usually range in size from a few millimeters to several centimeters, and they can be round or oval in shape. They have a large base or a tiny stalk that attaches them to the uterine wall.
Endometrial polyps come from the lining, which is likewise delicate and mostly composed of endometrial tissue, in contrast to uterine fibroids, which originate from the uterine muscle layer. Endometrial polyps typically affect women between the ages of 40 and 50, although they can also affect younger women and women who have gone through menopause. Fertility work-ups and imaging tests may unintentionally reveal asymptomatic polyps.
Who is Affected by Uterine Polyps?
Uterine polyps can affect women of any age, but they are more commonly seen in women between 40 and 50 years of age, especially around perimenopause and menopause. Women with obesity, high blood pressure, hormonal imbalances, PCOS, or a history of fertility issues may also have a higher risk of developing uterine polyps.
Types of Uterine Polyps
There are majorly 6 types of uterine polyps. Let’s understand them briefly.
Endometrial Polyps: This kind develops in the inner lining of the uterus which is also known as endometrium. They are the most common type of uterine polyps.
Cervical Polyps: These grow on the surface or inside the cervical canal which connects the uterus to the vagina.
Sessile Polyps: These are flat polyps attached directly to the uterine wall without a stalk.
Pedunculated Polyps: Polyps attached to the uterine lining by a thin stalk or stem.
Benign Polyps: These are non-cancerous growths that make up the majority of uterine polyps.
Precancerous or Cancerous Polyps: These are rare types of polyps which can contain abnormal or cancerous cells, which is mostly common after menopause.
What are the Symptoms of Uterine Polyps?
Symptoms are not usually present with endometrial polyps. The most typical sign when they do is irregular uterine bleeding, which might include:
- After menopause, vaginal bleeding or spotting
- heavy flow when you're menstruating
- Longer than seven-day periods
- Spotting or bleeding in between periods
- Menstrual periods that are irregular
- After intercourse, bleeding
Infertility and pelvic discomfort are two more uncommon symptoms. A fertilized egg may not be able to implant in your uterus if you have polyps.
What Causes Uterine Polyps?
Endometrial polyps are thought to be caused by hormonal imbalances, mainly excessive oestrogen stimulation of the uterine lining, though the exact origin is yet unknown. Because estrogen causes the endometrium to grow, aberrant localized overgrowths may develop if estrogen levels remain high for an extended length of time without progesterone balancing impact.
Among the contributing elements are:
- Hormone fluctuation, especially with regard to the transition to perimenopause.
- Obesity, which is increased synthesis of estrogen, can result from excess body fat.
- Diabetes and hypertension are some metabolic diseases that may be linked to abnormal endometriosis.
- Long-term exposure to estrogen increases the risk for women over 40.
- Tamoxifen raises the risk and is used as a therapy for breast cancer. These kinds of abnormal growths may also be more common in women who have used hormone therapy, had irregular periods, or suffered infertility.
Are Uterine Polyps Dangerous?
Uterine polyps are generally non-cancerous growths that develop in the lining of the uterus. While most are harmless in nature, some cases lead to symptoms such as heavy periods, irregular bleeding, or fertility issues. There are some rare cases in which these uterine polyps can contain precancerous or cancerous cells, hence, on-time medical evaluation is a must.
How are Uterine Polyps Diagnosed?
Uterine polyps may be diagnosed using the following tests:
Ultrasound: When inserted into the vagina, a thin, wand-like instrument produces sound waves and an image of the uterus, including its interior. There may be a region of thicker endometrial tissue or an obvious polyp.
Hysterosonograph: Also known as sonohysterography, it is a related technique in which a tiny tube inserted via the vagina and cervix is used to inject salt water (saline) into the uterus. During the ultrasound, the saline causes the uterus to enlarge, making the inside of the uterus more visible.
Hysteroscopy: In this a thin, flexible, illuminated telescope (hysteroscope) is inserted into the uterus via the cervix and vagina. Hysteroscopy makes it possible to see within the uterus.
endometrial biopsy. A sample is taken for laboratory analysis using a suction catheter placed into the uterus. An endometrial biopsy may confirm uterine polyps, however it may also miss the polyp.
How Uterine Polyps (Endometrial Polyps) Affect Fertility
Here are some ways in which uterine polyps affect fertility:
- It obstructs the sperm from migrating through the cervical canal or uterine cavity implantation.
- Due to the changes in endometrial receptivity, the success embryo implantation rate reduces.
- Chronic inflammation can hamper the growth of embryos.
- Endocrine disorders lead to inadequate endometrial care, particularly during the luteal phase.
Hysteroscopic polypectomy can greatly improve pregnancy outcomes, according to clinical experience, especially for individuals undergoing assisted reproductive treatments like in vitro fertilization (IVF) or intrauterine insemination (IUI).
Complications of Uterine Polyps
- Irregular menstrual bleeding or spotting between periods
- Heavy or prolonged menstrual flow
- Difficulty in conceiving or fertility-related issues
- Pelvic pain or discomfort in some cases
- Increased risk of iron deficiency anaemia due to heavy bleeding
- Possibility of recurrence after treatment
- Rare risk of precancerous or cancerous changes in the polyp
How are Uterine Polyps Treated?
Uterine polyps may be treated with:
- Without any symptoms, small polyps may go away on their own. People who are not at risk of uterine cancer do not need to treat tiny polyps.
- Progestins and gonadotropin-releasing hormone agonists are two examples of hormonal drugs that may reduce polyp symptoms. However, using such drugs is usually only a temporary fix; once the drug is withdrawn, symptoms frequently return.
- Polyps can be removed during hysteroscopy by inserting devices through the hysteroscope, a device used to examine within the uterus. The resected polyp will probably be sent to a laboratory for analysis.
