What is an Ectopic Pregnancy?

A pregnancy that occurs outside of your uterus is known as an ectopic pregnancy. When a fertilized egg implants in an area that cannot sustain its growth, this happens. The fallopian tube, which connects your uterus and ovaries, is where an ectopic pregnancy typically occurs. Rarely, ectopic pregnancies can develop in the cervix, abdomen, or ovary. Because only your uterus is designed to carry a pregnancy, ectopic pregnancies cannot proceed. Ectopic pregnancies have the potential to be fatal, particularly if your fallopian tube ruptures. This is an ectopic pregnancy that has ruptured, which can result in serious bleeding, infection, and occasionally even death. This is an urgent medical situation. Ectopic pregnancies must be promptly treated by medical professionals.

Where Does an Ectopic Pregnancy Happen & How Common is it?

As already mentioned above, an ectopic pregnancy happens outside of the uterus. The most common location is the fallopian tube, as it is the first structure the fertilised egg passes through after leaving the ovary. In rarer cases, the fertilised egg may implant in the ovary, the cervix, or even within the abdominal cavity. Each of these locations presents its own set of risks, as none of them can support the growth of a developing embryo. Regardless of where the implantation occurs, prompt medical evaluation is essential to prevent life-threatening complications. Ectopic pregnancies are very common and they occur in about 2% of all pregnancies.

What Are the Different Types of Ectopic Pregnancy?

Tubal Ectopic Pregnancy

This is the most common type of ectopic pregnancy. In this condition, the fertilised egg implants inside the fallopian tube instead of the uterus.

Ovarian Ectopic Pregnancy

In rare cases, the fertilised egg attaches to the ovary and starts developing there.

Cervical Ectopic Pregnancy

This occurs when the embryo implants in the cervix, which is the lower part of the uterus connecting to the vagina.

Abdominal Ectopic Pregnancy

In this type, the fertilised egg implants somewhere inside the abdominal cavity outside the reproductive organs.

Cesarean Scar Ectopic Pregnancy

A rare form of ectopic pregnancy where the embryo implants into the scar tissue from a previous C-section surgery.

Heterotopic Pregnancy

This is a rare condition in which one pregnancy develops inside the uterus while another develops outside the uterus at the same time.

What Are the Symptoms of an Ectopic Pregnancy?

Early ectopic pregnancy symptoms can resemble normal pregnancy symptoms. However, during an ectopic pregnancy, you might also have the following symptoms:

  • bleeding from the vagina
  • lower back, pelvic, and lower abdominal pain
  • weakness or dizziness

The pain and bleeding from a fallopian tube rupture may be severe enough to result in other symptoms. These may consist of:

  • losing consciousness
  • hypotension, or low blood pressure
  • soreness in the shoulder
  • bowel issues or rectal pressure

You may experience abrupt, intense lower abdomen discomfort when a tube bursts. This is an urgent medical situation. You should visit the emergency department or get in touch with your healthcare practitioner right away.

Ectopic Pregnancy Signs Week by Week

The symptoms for this condition can vary from one woman to another. There are cases in which the symptoms appear in the early stage itself, however, in some cases women do not notice symptoms until the pregnancy progresses. Hence, understanding the signs week by week can help seek medical care when needed.

Weeks 4-5

During the early stages, women might feel the symptoms are similar to that of a normal pregnancy. Here are some signs you can look out for: 

  • Missed periods
  • Positive pregnancy test
  • Fatigue or nausea
  • Light spotting
  • Mild pain or discomfort in the lower abdominal area

Weeks 5-6

As the ectopic pregnancy grows, the symptoms can become noticeable. Here are some of the common signs:

  • Pelvic or abdominal pain on one side
  • Increased vaginal spotting
  • Cramping
  • Pain during physical activity

Weeks 6-8

At this stage, the symptoms become evitable which can lead to:

  • Shoulder tip pain
  • Dizziness
  • Heavier vaginal bleeding
  • Severe pain in the abdominal area
  • Pain while using the toilet

Weeks 8 and Beyond

If right medical care is not received by now, then the symptoms can increase. Look out for:

  • Abdominal pain that is sudden and severe
  • Collapsing or fainting
  • Quick heartbeat
  • severe bleeding inside the body
  • Severe weakness or perplexity

What Causes an Ectopic Pregnancy?

A fertilized egg's inability to swiftly pass through the fallopian tube and enter the uterus is typically the cause of an ectopic pregnancy. An infection or inflammation may obstruct the tube. The following may obstruct the tube:

  • Pelvic inflammatory disease (PID)
  • Endometriosis, which occurs when uterine lining cells implant and proliferate in other parts of the body because to scar tissue from prior fallopian or abdominal operations
  • Rarely, congenital abnormalities that altered the tube's form 

How Is an Ectopic Pregnancy Diagnosed?

A doctor may do an ultrasound on a woman who may have an ectopic pregnancy to determine the location of the growing fetus. Until more than five or six weeks after a woman's last menstrual cycle, pregnancies are frequently too small to be seen on ultrasound. The doctor may use a wand-like instrument in the vagina to do the test if an external ultrasound is unable to detect the pregnancy. If the initial tests are unable to establish or rule out an ectopic pregnancy, a woman may require testing every few days.

What are the Complications of an Ectopic Pregnancy?

Here are some complications of an ectopic pregnancy:

  • Rupture of the Fallopian Tubes- As the fertilised egg grows, it can cause the tube to stretch and eventually tear. This can lead to a sudden and severe internal bleeding. 
  • Internal Bleeding- Significant internal bleeding into the abdominal cavity may result from an ectopic pregnancy rupture. This can cause shock, a severe drop in blood pressure, and even death if treatment is not received. 
  • Emotional or Psychological Impact- Apart from the physical complication, an ectopic pregnancy can also take a significant toll on your emotions. Grief, anxiety, and sadness can result from the loss of a pregnancy as well as the stress and anxiety of a medical emergency. During this period, seeking counseling and emotional assistance is just as crucial as physical recuperation. 

How is an Ectopic Pregnancy Treated?

In order to avoid a potentially fatal rupture, an ectopic pregnancy must be managed since it cannot be carried to term. The location, the woman's general health, and the stage of the pregnancy all influence the course of treatment.

  • Medication: A methotrexate injection may be used by a physician to stop the growth of pregnancy tissue when an ectopic pregnancy is identified early with low hCG levels. This helps prevent surgery and safeguards the fallopian tube by enabling the body to absorb it naturally. 
  • Surgery: The main treatment for an ectopic pregnancy is surgery. To remove the ectopic tissue, a surgeon typically uses a minimally invasive technique known as laparoscopy, or keyhole surgery. A little incision is typically made close to the belly button, and the afflicted tissue is seen and removed using a thin, illuminated tool. Sometimes it's also necessary to remove the afflicted fallopian tube. A larger incision, called a laparotomy, may be necessary in urgent or complex instances. 

Recovery and Future Fertility

It takes time to move on from the loss of an ectopic pregnancy, both physically and emotionally. The recovery period may vary from woman to woman and also on the type of treatment received (medication or surgery). Some women recover within a couple of weeks, however, some need more time to heal completely. It is recommended to keep the hCG levels on bay which is monitored weekly. Take rest, and avoid any strenuous activities during this time until your body heals properly. After an ectopic pregnancy, many women are able to conceive naturally, particularly if the issue is identified and treated promptly. The state of the fallopian tubes, the underlying reason of the ectopic pregnancy, and general reproductive health are some of the variables that frequently affect future fertility.
