What is a Unicornuate Uterus?

It is a rare congenital condition in which only one side of the uterus develops fully, resulting in a smaller uterine cavity. While a unicornuate uterus pregnancy may carry certain risks and require closer monitoring. Many women still go on to have successful pregnancies with the right medical care and support. Understanding the condition, its impact, and available treatment options can help you make informed decisions about your reproductive health.

Types of Unicornuate Uterus

Here are some of the common unicornuate uterus types:

  • Unicornuate uterus without a rudimentary horn: In this type only one side of the uterus develops, and no rudimentary horn is present.
  • Unicornuate uterus with a non-communicating rudimentary horn: A rudimentary horn is present but does not connect to the main uterus.
  • Unicornuate uterus with a communicating rudimentary horn: A rudimentary horn is present and connected to the main uterus.
  • Unicornuate uterus with a functional rudimentary horn: The rudimentary horn contains functional endometrial tissue, which may lead to symptoms and complications.

What are the Symptoms of a Unicornuate Uterus?

Before puberty, children born with a unicornuate uterus typically show no symptoms. In certain instances, they may also have additional health problems, like kidney problems. While treating those disorders, doctors could find the unicornuate uterus. If there is a hemi-uterus in addition to the unicornuate uterus, the diagnosis of the unicornuate uterus is more frequently made when an adolescent experiences menstrual pain or discomfort. When they want a child, some people find out they have a unicornuate uterus. Their uterus's peculiar form may make it difficult for them to conceive. When they do become pregnant, a simple ultrasound may reveal their unicornuate uterus.

What are the Causes of a Unicornuate Uterus?

A unicornuate uterus is a congenital uterine abnormality that develops before birth, during the formation of the female reproductive system. It occurs when one of the two Müllerian ducts does not develop properly or develops only partially, resulting in a smaller, single-sided uterus.

The causes of a unicornuate uterus includes:

  • Abnormal Müllerian Duct Development: The uterus normally forms from two Müllerian ducts that fuse together during fetal development. A unicornuate uterus results when one duct fails to develop completely.
  • Congenital Condition: It is present from the birth and develops while the baby is still in the womb.
  • Genetic or Development Factors: Unicornuate uterus causes also because of certain genetic or embryological developmental abnormalities.
  • Associated Kidney Abnormalities: In some women, a unicornuate uterus may occur alongside kidney abnormalities because the urinary and reproductive systems develop together in the fetus.
  • Formation of a Rudimentary Horn: Due to the partial development of the second Müllerian duct can lead to rudimentary horn, which might not connect to the main uterus

How Does a Unicornuate Uterus Impact Fertility and Pregnancy?

A unicornuate uterus can present challenges for both fertility and pregnancy. The smaller size and shape of the uterus can interfere with conception and carrying a pregnancy to term.

Impact of Unicornuate Uterus on Fertility

An embryo may find it more difficult to implant in the uterine lining since the uterus is smaller and has a different shape. In order to increase the odds of success, this may result in longer times spent attempting to conceive or the requirement for reproductive procedures like In Vitro Fertilization (IVF)

Impact of Unicornuate Uterus on Pregnancy

  • Increased chance of miscarriage: The smaller uterus may not offer enough room for the developing baby if a pregnancy is confirmed. This may make miscarriage more likely, especially in the early stages of pregnancy. Before becoming pregnant, women with a unicornuate uterus may have several miscarriages.
  • Early birth: Women with this illness are more likely to give birth before 37 weeks of pregnancy because of the restricted space in the uterus. Close observation and medical attention are crucial to reducing the risk of health problems for preterm infants.
  • Ectopic pregnancy: Pregnancy may occasionally take place in the rudimentary horn, an underdeveloped area of the uterus. An ectopic pregnancy is a dangerous kind of pregnancy that needs to be treated right away. A developing pregnancy cannot be supported by the crude horn, which puts the mother's health at risk.

Unicornuate Uterus Pregnancy Success Rate

The degree of the disease, the existence of a primitive horn, and general reproductive health all affect pregnancy success rates. Many women can have successful pregnancies with early diagnosis, specialized prenatal care, and routine monitoring.

Unicornuate Uterus IVF Success Rate

For women who are having trouble conceiving due to a unicornuate uterus, IVF can increase the likelihood of pregnancy. Age, egg quality, embryo health, and uterine condition are some of the characteristics that typically affect success rates. Even after a successful IVF conception, careful pregnancy monitoring is still necessary.

How Are Periods Affected by a Unicornuate Uterus?

Common ways a unicornuate uterus may affect periods include:

  • Painful periods (dysmenorrhea): Menstrual pain can occur due to restricted blood flow or buildup of menstrual blood in a non-communicating rudimentary horn.
  • Irregular menstrual cycles: Some women may experience irregular or unpredictable periods.
  • Heavy menstrual bleeding: In certain cases, periods may become heavier than usual.
  • Pelvic pain: Chronic pelvic discomfort may occur, especially if the rudimentary horn is functional.
  • No noticeable symptoms: Many women with a unicornuate uterus have normal periods and may only discover the condition during fertility or pregnancy evaluations.

How is the Diagnosis of a Unicornuate Uterus Made?

When a woman suffers repeated losses or struggles to conceive, a unicornuate uterus is frequently found.

The following are a few tests that are used to diagnose this condition:

  • The most popular method for seeing the size and shape of the uterus is ultrasound.
  • Unicornuate Uterus Hysterosalpingogram (HSG): To determine the shape of the uterine cavity and determine whether the fallopian tubes are obstructed, a dye is injected into the uterus and then X-ray imaging is performed.
  • Magnetic resonance imaging (MRI): This method is occasionally used to see the uterus and surrounding organs in greater detail.

What are the Tests Performed During Diagnosis?

A unicornuate uterus is generally diagnosed when couples go for fertility evaluations or pregnancy scans. It can also be diagnosed during investigations for pelvic pain and recurrent miscarriage. Here are the tests that can be performed during the diagnosis:

  • Pelvic Ultrasoun: A transabdominal or transvaginal ultrasound helps identify the shape and size of the uterus and detect the presence of a rudimentary horn.
  • 3D Ultrasound: This helps provide a clear and detailed view of uterine abnormalities compared to a standard ultrasound.
  • Hysterosalpingography (HSG): An X-ray procedure where dye is injected into the uterus to examine the uterine cavity and fallopian tubes.
  • MRI Scan: MRI provides precise diagnosis confirmation by providing comprehensive imaging of the uterus and associated pelvic organs.
  • Laparoscopy: In certain situations, the uterine and pelvic tissues may be directly examined by a minimally invasive surgical procedure.
  • Kidney Evaluation: Doctors may also suggest renal imaging testing because uterine abnormalities can be linked to kidney problems.

Options for Unicornuate Uterine Treatment

Although the uterus's physical structure cannot be changed, a number of therapies can control symptoms and promote a successful pregnancy:

  1. Laparoscopy Surgery: By removing a primitive, non-communicating horn, this minimally invasive surgery lowers the chance of ectopic pregnancy and other consequences.
  1. Cerclage: A cerclage, or surgical stitch, helps strengthen the cervix and reduce the risk of premature birth in women who are at risk for cervical insufficiency.
  1. Progesterone Treatment: By lowering the chance of miscarriage, progesterone supplementation promotes uterine function and aids in maintaining a healthy pregnancy.
  1. Treatments for Fertility: Assisted reproductive technologies present viable answers if natural conception becomes difficult:
  • IUI: Beneficial for minor infertility problems.
  • IVF: By directly implanting embryos, it helps get beyond uterine restrictions.
  • ICSI :When uterine anomalies coexist with male factor reproductive issues, ICSI is the best option.
