What is Endometriosis?

The abnormal growth of internal uterine tissue outside the uterus is known as endometriosis. This tissue typically accumulates in the uterus every month and is shed during menstruation. On the other hand, endometriosis causes this tissue to grow outside the uterus, typically on the fallopian tubes, ovaries, and pelvic tissues.

With each menstrual cycle, this displaced endometrial tissue thickens, breaks down, and bleeds as it usually would inside the uterus. However, the blood and tissue expelled during menstruation cannot leave the body because it is situated outside the uterus. Inflammation, discomfort, the development of scar tissue (adhesions), and occasionally issues with conception can result from this.

What are the Different Stages of Endometriosis?

Based on the location, quantity, size, and depth of endometrial-like tissue implants, endometriosis is divided into four stages: Stage I (Minimal) to Stage IV (Severe). The American Society for Reproductive Medicine (ASRM) has established recommendations for this classification, which is usually established after a diagnostic laparoscopy

Stage I – Minimal Endometriosis

Stage one involves a few small, superficial implants that are typically only 5mm deep. During this stage, the implants are found on the ovaries or pelvic peritoneum. You might notice some symptoms; however, there is little to no scar tissue. The common treatments suggested at this stage are NSAIDs and hormone therapy. Surgery is rarely needed. 

Stage II – Mild Endometriosis

There are more lesions in this stage which can lead to scar tissue or mild fibrosis. Mild endometriosis generally affects the pelvic lining as well as the ovaries. The symptoms include paid during menstruation and sexual activity. Treatment recommended is minimally invasive laparoscopy and hormonal treatment. 

Stage III – Moderate Endometriosis

In stage three there are multiple deep implants, found in one or both ovaries. Also, known as chocolate cysts, the scar tissue or noticeable adhesions are frequently linked to persistent pelvic discomfort and infertility. To stop recurrence, surgical excision of cysts and adhesions is frequently necessary, followed by medication. 

Stage IV – Severe Endometriosis

The 4th stage involves thick adhesions, massive endometriomas, and extensive deep implants. Which impacts the ovaries, fallopian tubes, colon, bladder, and other pelvic organs. These impacts result in infertility, intestinal problems, and excruciating discomfort. One suffering from severe endometriosis may need major surgery and, if fertility is affected, assisted reproductive technologies (ART) like IVF

For example, a person with Stage IV may experience little to no discomfort, but a person with Stage I may have severe symptoms. Symptoms (less common and common symptoms)

The primary symptom of endometriosis is a pain in the lower abdomen and back. The pain gets worse before and during the periods. Painful Periods

Dysmenorrhea or painful periods is a prominent symptom of endometriosis. The pelvic pain and cramping usually begin before and last several days during the menstrual period. The pain extends typically from the lower abdomen to the back and thighs. The pain is generally cutting and stabbing in nature.

Painful Intracourse

Pain during sexual intercourse is common among women with endometriosis. 

Painful Urination

Women with endometriosis experience pain during bowel movements or urination. These symptoms usually occur during the menstrual period.

Heavy Menstrual Bleeding

Women with endometriosis experience heavy menstrual bleeding. Sometimes the women may also experience bleeding between periods.

Infertility

The risk of developing infertility is twice as high in women having endometriosis than a normal woman.

What are the Main Causes of Endometriosis?

The exact endometriosis causes are not fully known, but several theories and risk factors have been suggested:

Retrograde Menstruation

One of the most widely discussed hypotheses for the development of endometriosis is retrograde menstruation. This theory suggests that during menstruation, instead of all menstrual blood exiting the body through the vagina, some of it flows backwards through the fallopian tubes into the pelvic cavity. This reversed flow may carry endometrial cells, which can then attach to pelvic organs such as the ovaries, bladder, or bowel and begin to grow outside the uterus.

Genetic Predisposition

Endometriosis tends to run in families, suggesting a possible genetic link.

Immune System Disorders

Endometriosis causes also include a weakened immune system that may fail to recognise and destroy misplaced endometrial tissue.

Hormonal Imbalance

Oestrogen promotes the growth of endometrial tissue, which may worsen the condition.

To diagnose endometriosis the doctor may suggest you:

Physical exam

During a physical pelvic examination, the doctor uses a special medical tool speculum to see the inside of the vagina and cervix. During the examination, the doctor may also inspect your abdomen for any cysts or scars around the lower abdomen.

Ultrasound

To detect abnormal endometrial growth outside the inner lining of the uterus the doctor may advise transvaginal or abdominal ultrasound. The ultrasound helps the doctor to identify any abnormal growth. 

Laparoscopy

Sometimes when a physical examination or ultrasound fails to identify endometriosis then the doctor may perform a laparoscopy to view the abnormal endometrial lining growth. Once laparoscopy helps in diagnosing endometriosis the doctor can get the abnormal tissue growth removed in the same procedure.

What Surgical and Non-Surgical Treatment Options are Available?

Surgery and/or medication are the most effective treatments for endometriosis. The severity of the patient's symptoms and whether or not she is young that is, whether or not she still wants to conceive, are taken into consideration when determining the appropriate course of treatment for endometriosis.
