PCOS Treatment in India

PCOS stands for Polycystic Ovary Syndrome, which is one of the most common disorders related to hormones that affects millions of women during their reproductive period. Despite being often linked with irregular periods and infertility, PCOS may affect weight, skin, metabolism and overall well-being of a woman. The variability of symptoms makes it hard to notice that you have PCOS until you experience any problems with your menstrual cycle or fertility. The good news is that PCOS is manageable. With the right diagnosis, healthy lifestyle changes, and medical assistance, many women are able to control their symptoms and achieve a healthy pregnancy. Understanding the condition is the first step towards taking charge of your reproductive health.

What is PCOS?

PCOS is among the most prevalent hormone disorders in women of reproductive age. This condition is characterised by the excessive production of androgens, otherwise known as male hormones, by the ovaries that may lead to issues with ovulation and menstrual cycle regulation.

Notably, PCOS is not always accompanied by the formation of ovarian cysts despite its name. However, it can impact various aspects of women's health including menstrual cycles, fertility, metabolic functioning, skin health, and overall well-being. Symptoms and severity can vary significantly from one woman to another. With timely diagnosis and appropriate treatment, many women with PCOS can successfully manage their symptoms and achieve pregnancy.

Types of PCOS

While there is specific clinical criteria that is used to diagnose PCOS, healthcare providers often classify it into various patterns depending on the underlying hormonal or metabolic concerns.

Insulin-Resistant PCOS

This is the most common form of PCOS. In this type, the body's cells become less responsive to insulin, which leads to an increase in insulin levels. High insulin levels can stimulate the ovaries to produce more androgens, which may lead to irregular periods, weight gain and fertility issues.

Inflammatory PCOS

Chronic low-grade inflammation may contribute to hormonal imbalances in some women. The person may experience symptoms like fatigue, skin problems and elevated inflammatory markers.

Adrenal PCOS

In certain situations, increased androgen levels occur because of increased androgen production by the adrenal glands, not by the ovaries. The women suffering from adrenal PCOS may have high stress hormone levels together with the symptoms of increased androgen levels.

Post-Pill PCOS

Some women suffer from temporary PCOS symptoms after stopping their hormonal contraceptives. These symptoms may improve over time as hormone levels stabilise.

Common Signs & Symptoms of PCOS

Symptoms vary from person to person. Some women may experience only mild symptoms while others may face noticeable changes impacting their health.

Irregular or Missed Periods

Irregular menstruation is one of the key signs of PCOS. A woman with PCOS may have inconsistent menses which may remain absent for months or become unpredictable due to irregular ovulation.

Hirsutism

Excess androgen levels can lead to unwanted hair growth on the face, chin, chest, abdomen, or back. However, the extent of hair growth differs from person to person.

Acne and Oily Skin

Hormonal fluctuations can increase oil production in the skin. This may lead to prolonged acne that may continue even beyond the teenage years.

Weight Gain

Many women with PCOS find it difficult to maintain or lose weight. Weight gain often occurs around the abdomen and can further worsen insulin resistance.

Thinning Hair

Some women experience scalp hair thinning or increased hair shedding, particularly around the crown of the head.

Difficulty Getting Pregnant

Because PCOS can interfere with ovulation, it may make conception more challenging without treatment.

Darkened Skin Patches

Skin may get darker, particularly around the neck, underarms, or groin due to insulin resistance.

What Causes PCOS?

The exact cause of PCOS is still not clear but experts believe numerous factors contribute to its development.

Hormonal Imbalance

Individuals with PCOS have higher levels of androgens and an abnormal reproductive hormone pattern. These imbalances can result in problems with the development and release of eggs from the ovaries.

Genetics and Family History

PCOS also has a genetic link. Having a mother, sister, or close relative with PCOS may increase the chances of developing this condition.

Insulin Resistance

Insulin resistance is common in women with PCOS. In such a scenario, the cells of the body do not respond to insulin effectively. With a rise in insulin levels, the secretion of androgen may also increase, which causes PCOS symptoms.

Chronic Inflammation

Some studies have shown that low-grade inflammation may play a role in stimulating androgen production and worsening PCOS symptoms.

Lifestyle and Environmental Factors

While lifestyle alone does not cause PCOS, factors such as obesity, physical inactivity and dietary habits can impact symptom severity and disease progression.

How Does PCOS Affect Fertility?

Polycystic Ovary Syndrome (PCOS) is one of the leading reasons for female infertility. This is because PCOS can affect ovulation. Under normal circumstances, an egg is usually released by the ovary once a month. In cases where a woman suffers from PCOS, the hormonal imbalance may interfere with the release or the maturation of the egg.

Different women have different experiences when it comes to fertility issues resulting from PCOS. Some conceive normally despite suffering from PCOS, while others need assisted reproduction methods such as ovulation induction, IUI, or IVF. Early diagnosis and treatment can significantly improve the chances of pregnancy.

How is PCOS Diagnosed?

The diagnosis for PCOS may involve a combination of medical history, symptom assessment, physical examination, and diagnostic tests.

Your fertility specialist may recommend:

  • Detailed menstrual history
  • Assessment of symptoms such as acne, excess hair growth, and weight changes
  • Pelvic ultrasound to evaluate ovarian appearance
  • Blood tests to measure hormone levels
  • TSH and Prolactin testing to rule out other causes of irregular periods
  • Blood sugar and insulin assessments
  • Lipid profile testing

PCOS Treatment Options in India

Treatment for PCOS depends on a range of factors including symptoms, reproductive goals, age, and overall health of the woman.

Lifestyle Modifications

Lifestyle changes form the foundation of PCOS management for many women. These may include:

  • Regular physical activity
  • Weight management
  • Balanced nutrition
  • Stress reduction
  • Improved sleep habits

Even modest weight loss can help improve ovulation and menstrual regularity in some women.

Medications

Medication plans are personalised and may be prescribed by doctors to address specific symptoms and underlying hormonal imbalances. These may include:

  • Ovulation-inducing medications
  • Insulin-sensitising medications
  • Hormonal contraceptives for cycle regulation
  • Treatments for acne and excess hair growth

Fertility Treatments

If pregnancy is the goal, a fertility specialist can determine the most suitable treatment approach based on your age, ovarian reserve, and fertility history. Some of the fertility treatments that may be advised are

When Should You See a Fertility Specialist for PCOS?

  • Consistently irregular or absent periods
  • Been trying to conceive without success
