Types of Dysmenorrhea

Dysmenorrhea is categorised into two categories:

  • Primary Dysmenorrhea- The most frequent type of dysmenorrhea is primary dysmenorrhea, which occurs when there is no underlying medical issue. It usually starts shortly after a woman begins menstruation and lasts throughout her life. This type produces severe and frequent menstrual cramps as a result of irregular uterine contractions.
  • Secondary Dysmenorrhea- On the other hand, secondary dysmenorrhea occurs later in life and is linked to a specific medical condition. Common reasons include endometriosis, adenomyosis, fibroids, and pelvic inflammatory disease.

Symptoms of Dysmenorrhea

Dysmenorrhea causes severe menstrual pains that usually start 24 to 48 hours before menstruation. The pain, which is commonly described as throbbing or aching, is typically felt in the lower abdomen and can spread to the lower back, hips, and inner thighs. Women with dysmenorrhea may also suffer from:

  • Nausea and vomiting
  • Diarrhoea
  • Headaches
  • Fatigue

Causes of Dysmenorrhea

Every woman has different causes behind the unpleasant menstrual cramps. Here are the most common causes of dysmenorrhea:

Excess Prostaglandin Production

If a woman has an excess quantity of prostaglandin, then it causes the uterus to contract more during menstruation which results in severe cramps. This is also one of the main causes behind primary dysmenorrhea.

Endometriosis

This is the condition where tissues alike to that of uterine lining grow outside the uterus which results in severe monthly discomfort, excessive bleeding, and infertility.

Adenomyosis

Adenomyosis occurs when the uterine lining grows into the muscular wall of the uterus, causing painful periods, heavy menstrual bleeding, and an enlarged uterus.

Uterine Fibroids

Fibroids are non-cancerous growths in or around the uterus that, depending on their size and location, can cause severe menstrual cramps, prolonged periods, and pelvic pain.

Pelvic Inflammatory Disorder (PID)

PID is a female reproductive organ infection that, if not treated, can cause pelvic pain, unpleasant periods, abnormal vaginal discharge, and fertility issues.

Uterine abnormalities

Congenital anomalies such a septate uterus or cervical stenosis can disrupt normal menstrual flow, increasing the risk of unpleasant periods.

Ovarian cysts

Certain ovarian cysts, especially large or burst cysts, can cause pelvic pain, which worsens during menstruation.

Diagnosis of Dysmenorrhea

Dysmenorrhea is diagnosed in stages, beginning with a detailed discussion of your symptoms and continuing to more specific tests as needed. Here's how doctors usually arrive at a diagnosis:

  • Medical History Review: Diagnosing dysmenorrhea entails thoroughly evaluating symptoms and medical history. Doctors usually start with a thorough examination of menstrual cycles, pain patterns, and related symptoms.
  • Physical and Pelvic Examination: Gynaecologists frequently undertake a pelvic examination to detect problems in the reproductive organs.
  • Pelvic Ultrasound: In rare circumstances, additional testing may be required to identify the underlying causes of dysmenorrhea. These may include ultrasound scans that produce images of the uterus and ovaries.
  • Additional Scans: More complex instances may require advanced imaging techniques such as CT scans or MRIS. Rarely, the doctor may request a laparoscopy to directly inspect the pelvic organs and diagnose underlying issues causing severe menstruation discomfort.

Dysmenorrhea Treatment Options

Dysmenorrhea pain treatment focuses on treating both symptoms and underlying causes. Here are the most common dysmenorrhea treatment options:

Pain Relief Medications (NSAIDs)

The most common option that is suggested by doctors are pain relief medications, like NSAIDs. These include medications like ibuprofen and naproxen, which can be purchased at your local drugstore or grocery store. They act by reducing prostaglandin levels in the body. It's best to take them as soon as the cramps start. If you are unable to take NSAIDs, you may choose an alternative pain reliever, such as acetaminophen.

Hormonal Therapy

Oral birth control pills contain substances that inhibit ovulation and lessen the severity of menstrual cramps. These hormones can also be supplied in a variety of other ways, including an injection, a skin patch, an implant placed beneath the skin of your arm, a flexible ring inserted into your vagina, or an intrauterine device.

Surgery

If your menstrual cramps are caused by a disorder like endometriosis or fibroids, surgery to treat the condition may alleviate your discomfort. If various treatments fail to alleviate your discomfort and you do not intend to have children, surgical uterine removal may be a viable option.

Lifestyle Modifications

Lifestyle changes including having a balanced diet and including exercise in the daily routine does help with decreasing the symptoms of dysmenorrhea. Regular physical activity, particularly aerobic exercise, can assist to alleviate the severity of menstrual cramps by increasing blood circulation and decreasing tension. A well-balanced diet, enough hydration, and a reduction in caffeine, alcohol, and salty foods may help relieve symptoms.
