What is a Polyp?

A polyp is described as an unusual tissue growth developing from the inner lining of an organ. The condition results when cells from the lining multiply beyond normal, producing a soft, projecting growth. Polyps come in various sizes and shapes, some being small like seeds while others become considerably bigger.

Polyps are mostly benign growths. Some polyps are small and asymptomatic while others become symptomatic and result in abnormal bleeding, menstrual disturbances or infertility. In very rare instances, a polyp can become precancerous or cancerous, making timely evaluation important. 

The location, size, and number of polyps are the factors that determine whether treatment is needed. Your doctor may suggest regular monitoring for small, symptom-free polyps or removal if they are causing symptoms, affecting fertility or appear suspicious.

What Are the Types of Polyps?

Polyps can develop in different organs throughout the body and are classified based on where they occur and how they are attached to the surrounding tissue.

Colon Polyps

The polyps colon are those that form inside the large intestine. Even though most of them are harmless, there are some cases where they can become cancerous if not treated.

Nasal Polyps

These types of polyps are soft, and they are found either inside the nose or the sinuses. The nasal polyps are mostly associated with chronic inflammation or allergies.

Stomach (Gastric) Polyps

Stomach polyps occur on the lining of the stomach. They are normally identified through an endoscopy, and they are mostly benign.

Gallbladder Polyps

Gallbladder polyps are growths on the inner wall of the gallbladder. Most are harmless although larger polyps may require treatment.

Uterine Polyps

Also known as the endometrial polyps, these polyps develop from the lining of the uterus. They may cause abnormal uterine bleeding, menstrual irregularities or female infertility.

Cervical Polyps

A cervical polyp (or cervix polyp) is a small growth that develops on the cervix. These polyps are usually benign but may cause spotting, abnormal bleeding, or vaginal discharge.

Types of Polyps Based on Shape

Pedunculated Polyp

A pedunculated polyp is attached to the tissue by a thin stalk, which makes it resemble a mushroom. This type is generally easier to remove.

Sessile Polyp

A sessile polyp has a broad and flat base and is attached directly to the tissue surface. Depending on its location, it may require more careful removal than a pedunculated polyp.

There are some hormones that are responsible for causing the development of some polyps, especially those in the uterus. For instance, increased oestrogen levels can stimulate excessive growth of the uterine lining leading to polyps. 

Increasing Age

As people grow older, the chances of developing polyps become higher. Polyps in the uterus and colon mostly develop in people who are above 40 years old.

Genetic Factors

Some inherited conditions can increase a person's risk of developing multiple or recurrent polyps, particularly in the colon.

Obesity and Lifestyle Factors

High body weight, high blood pressure, and diabetes may increase the chances of developing some types of polyps.

How Are Polyps Diagnosed?

Diagnosis is important to find out whether a polyp requires monitoring or treatment. Your doctor may suggest one or more of the following investigations:

Medical History 

Your doctor will review your symptoms, medical and family history, and may perform a physical examination of the pelvis, nose, or abdomen, depending upon the location of the suspected polyp. 

Imaging Tests

There are different imaging tests like ultrasound, CT scan or MRI that may be suggested based on the type of polyp being evaluated. In cases where a patient may have a uterine polyp, the first imaging test that can be advised is the transvaginal ultrasound to analyse the size and exact location of the polyp.

Endoscopy

The endoscope is a thin and flexible tube with a camera that helps medical practitioners visualise the inside of an organ. The kind of endoscopy performed is determined by the place where the polyp is found, including colonoscopy in case of colon polyps, nasal endoscopy when the polyp is located in the nose, gastroscopy for stomach polyps, or hysteroscopy for polyps affecting the uterus.

Saline Infusion Sonography (SIS)

If a uterine polyp is suspected, your doctor may recommend saline infusion sonography (SIS). During this procedure, sterile saline is introduced into the uterus during an ultrasound to provide a clearer view of the uterine cavity and improve the detection of endometrial polyps.

Biopsy

When necessary, your doctor might take out a small sample of the tissue from your body or the whole polyp for a lab test. Biopsy will help diagnose the problem and determine if the polyp is benign, precancerous or cancerous. Women having uterine polyps can be suggested an endometrial biopsyin selected cases, particularly after menopause or when there is a higher risk of endometrial cancer.

How is a Polyp Treated?

Polyp treatment will depend on the position, size, symptoms, and whether there is a risk of cancer. While some small-sized and asymptomatic polyps may just need regular monitoring, others may have to be removed surgically or managed with medications.

Endometrial Polyp Removal

Endometrial polyp removal is recommended when a uterine polyp causes abnormal bleeding, infertility, recurrent pregnancy loss, or appears abnormal on imaging. Removing the polyp also allows it to be examined under a microscope to rule out precancerous or cancerous changes.

Hysteroscopic Polypectomy

Hysteroscopic polypectomy is the preferred treatment for uterine polyps. During this minimally invasive procedure, a hysteroscope is passed through the cervix to locate and remove the polyp without making any external incisions. It is a safe and effective procedure that helps preserve the healthy uterine lining and is commonly performed as a day-care surgery.

How Can Polyps Be Prevented?

There are no guaranteed ways of preventing polyps since different factors determine their occurrence. Nevertheless, living a healthy lifestyle, having an effective treatment of any other existing medical condition, and undergoing health screening on a regular basis may help reduce the chances of developing some types of polyps and enable early detection.

  • The steps that may be taken are:
  • Undergo recommended health screenings based on your age and medical history.
  • Do not ignore any symptoms such as abnormal bleeding, unexplained pain, alterations in bowel movements, blocked nose, and abnormal discharge.
