Anejaculation Treatment in India

Anejaculation is not something most men talk about easily. For many, it only becomes noticeable when they are trying for a pregnancy and things are not going as expected. It can affect both sexual health and fertility, but the concern often stays unspoken for a long time. Over the past few years, more men in India have started seeking help for fertility-related issues. This has made it easier to access the right tests and treatment. In most cases, doctors first try to understand what is causing the issue, because it is not the same for everyone. Once there is clarity, the next steps become much easier to plan.

What is Anejaculation?

Anejaculation simply means that semen is not released during orgasm. A man may still have desire and may also get a normal erection, but when it comes to ejaculation, it does not happen the way it usually should. Some men say they reach climax but there is no discharge at all. Others feel that something is missing, as if the orgasm is not complete. It may happen once in a while for some, while for others, it becomes a repeated issue.

For many men, this can be frustrating and sometimes confusing, especially when there are no other noticeable symptoms. The condition may affect confidence, relationships, and fertility plans, making it important to understand why it is happening and whether medical attention is needed.

It is important not to confuse anejaculation with erectile dysfunction. Here, the erection is usually not the problem. The difficulty is in the release of semen. In a few cases, semen does get released, but instead of coming out, it goes back into the bladder - a condition known as retrograde ejaculation. It can occur due to multiple reasons ranging from health conditions to medicines or even stress, that require evaluation.

Symptoms of Anejaculation

Symptoms of anejaculation can vary from person to person; however, the common ones include:

  • No release of semen during orgasm
  • Weak or incomplete orgasm
  • Ejaculation happens only in certain situations (for example, during masturbation but not intercourse)
  • Difficulty achieving pregnancy despite regular intercourse
  • Growing frustration, stress, or hesitation during intimacy

What Causes Anejaculation in Men?

There is no single cause of anejaculation, and the reasons can differ for every individual. There can also be more than one cause that might be creating this condition. Below is a list of common causes of anejaculation:

  • Nerve-related issues: If the nerves that control ejaculation are affected, the signal is not clearly passed, and ejaculation may not occur. People with diabetes, spinal injury, or a history of surgeries around the pelvic area are at high risk.

  • Certain medicines: Medicines taken during treatments for depression, high blood pressure, or prostate problems can sometimes interfere with ejaculation. Your doctor will review this to make adjustments to your medicines.

  • Hormonal problems: When testosterone levels are lower than normal, the body may not respond the same way during sexual activity and require medical intervention.

  • Psychological factors: There are also situations where the body is physically fine, but the mind gets in the way. Ongoing stress, anxiety, or even pressure to perform can disrupt the natural flow.

  • Lifestyle factors: Lifestyle habits also play an important role in reproductive health management. If there is high alcohol consumption, poor overall health, or long-term illness, it can slowly impact sexual health.

Types of Anejaculation

Anejaculation can be of various types and mainly categorised as:

Primary anejaculation: In this type, ejaculation has never happened and is often linked to long-standing or developmental issues. A comprehensive evaluation becomes important here.

Secondary anejaculation: Ejaculation has become difficult over time, and it was normal earlier. Underlying health conditions, certain medicines or lifestyle factors can be the reasons.

Situational anejaculation: Ejaculation occurs only in certain situations. For example, it may occur during masturbation but not during intercourse. It is mostly linked with psychological or situational factors.

Complete anejaculation: Ejaculation is completely absent, regardless of the situation or level of stimulation. This may point towards a stronger physical cause.

Retrograde ejaculation: A condition where semen flows back into the bladder instead of coming out. It may not be visible externally and can be validated through tests.

Diagnosis of Anejaculation

The diagnosis of anejaculation may involve a combination of the following assessments:

  • Medical History: Your doctor will ask you about any ongoing treatment or any previous health issues that might be impacting your reproductive health.
  • Physical examination: This is a simple assessment to check for any visible or structural problems in the reproductive organs.
  • Hormonal analysis: This is done through blood tests and primarily checks for testosterone and other hormone levels that impact sexual function.
  • Urine analysis: It helps detect the presence of sperm in urine to rule out retrograde ejaculation.

  • Additional tests if required: If stress or emotional factors are suspected, counselling can be suggested. If a nerve-related issue is suspected, tests to check nerve function or overall reproductive health can be advised.

Impact of Anejaculation on Fertility

For achieving a successful pregnancy, sperm have to fertilise the egg. If sperm is not released during ejaculation, as in the case of anejaculation, then fertilisation will not happen and pregnancy becomes difficult. That is why, if not treated, anejaculation can affect fertility, but with timely intervention, it can be managed and pregnancy is possible, especially using advanced techniques like sperm retrieval methods, where the sperm is retrieved and fertilised with an egg using IVF.

Treatments for Anejaculation in India

There is no fixed approach to treating anejaculation, as the cause of this condition can vary from person to person. Basis your unique case, your fertility specialist may recommend any of the following approaches

  • Adjustment of medicines: If certain medicines are affecting ejaculation, doctors may review them and suggest a change or adjustment.

  • Treating hormonal issues: When hormone levels, especially testosterone, are lower than expected, treatment may be advised to restore balance and improve sexual response.

  • Treating underlying conditions: In case of any health issues like diabetes or nerve-related problems, they are managed alongside since they can directly interfere with the ejaculation.

  • Assisted ejaculation methods: In some cases, medical techniques may be used to help trigger ejaculation or to collect sperm when natural release is not possible.

  • Medical counselling: If psychological factors such as stress, anxiety, or performance pressure are the reasons, counselling can help address these concerns and improve overall confidence.

  • Fertility treatment options: Doctors may suggest methods to retrieve sperm and use it in assisted reproductive procedures such as IVF, for couples trying for pregnancy.

  • Lifestyle changes: Simple changes in lifestyle habits can truly make a difference. Reduced alcohol consumption, avoiding smoking, and managing long-term health conditions can support your treatment and overall health.

Conclusion

Male infertility problems are common, and treatments are available. Anejaculation can feel worrying, especially when the reason is not known. But, in most cases, it is effectively managed after identification of the root cause and a personalised treatment plan. With timely medical support, things can be made better.
