For patients with high order multiple pregnancy, it is recommended to reduce the number of fetuses to two in order to reduce the risks of multiple pregnancy to both mother and fetuses. Embryo reduction has been shown to dramatically improve the outcome of the remaining fetuses and allows for a healthier pregnancy.

Why is Embryo Reduction Advised

Fertility treatments like IVF and IUI are known to increase the likelihood of having multiple pregnancy. As the number of fetuses increases, the risks of multiple pregnancy like preterm labour, low birth weight and impaired fetal development also increases. Embryo reduction is advised to make the pregnancy safer and healthier in pregnancies with three or more fetuses.

The Embryo Reduction Process

Embryo reduction can be done using a transvaginal approach between 7-9 weeks of pregnancy or using a transabdominal approach between 11-13 weeks of pregnancy. Both approaches use ultrasound guidance to visualise the embryos in the uterus during the procedure. A thin needle is then inserted into the uterus to inject embryos with chromosomal abnormalities (selective embryo reduction) or excess embryos (supernumerary embryo reduction) with a drug to reduce them.

Embryo reduction is generally done between 11 weeks to 13 weeks of the pregnancy.

Multiple pregnancies like twins, triplets, quadruplets etc are considered to be high risk and increase the likelihood of pregnancy complications including premature birth, miscarriages, preeclampsia, low birth rate and stillbirth.

You would be kept under observation for about half an hour once the procedure is completed and fetal heart rate is monitored. You may experience slight spotting and mild cramping for a few hours after the procedure and patients are advised to refrain from strenuous activity and exertion for a few days after the procedure.
