Septate Uterus Treatment

A septate uterus is a kind of congenital abnormality of the uterus, which arises due to incomplete disappearance of the tissue that separates the uterus into two parts during the development of the fetus. As a consequence, there forms a band of fibrous or muscular tissue called the uterine septum, which remains located in the uterine cavity, dividing it partially or entirely. There are many instances where females with a septate uterus have no symptoms at all and learn about their condition during fertility assessments or in case of recurrent miscarriages. The problem with a septate uterus is that it can interfere with implantation, restrict the space required for the baby’s development or increase the risk of miscarriage. A reassuring thing is that septate uterus treatment is available and can significantly improve reproductive outcomes in many cases. Modern minimally invasive procedures help restore the normal shape of the uterus while preserving fertility.

Types of Uterine Septum

A uterine septum can be classified on the basis of how far the tissue extends into the uterine cavity. Identifying the type helps figure out the most appropriate treatment approach.

Partial Septate Uterus

As the name suggests, the septum in this case extends only partway into the uterine cavity and does not reach the cervix, dividing the uterus partially. Many women have no symptoms, while others may experience infertility or recurrent miscarriage depending on the size of the septum.

Complete Septate Uterus

A complete septate uterus, the division starts from the top of the uterus and extends up to the cervix and, in some cases, into the vagina. This is a more complex type and can lead to infertility, recurrent miscarriage, preterm birth, and other pregnancy-related issues.

Septate Uterus vs Bicornuate Uterus

A septate uterus has a normal outer uterine shape with a tissue partition inside the cavity. Whereas, a bicornuate uterus has a heart-shaped outer contour, which happens because of incomplete fusion of the uterus during fetal development. Accurate diagnosis is important to plan the right treatment.

Septate Uterus vs Arcuate Uterus

An arcuate uterus is a mild variation in the shape of the uterus with a small indentation at the top of the uterine cavity. Unlike a septate uterus, it rarely affects fertility or pregnancy and usually does not require treatment.

Symptoms of a Septate Uterus

Women with a septate uterus may not exhibit any symptoms, and the problem may remain undiagnosed until they undergo fertility testing or pregnancy scans. But, depending on the size of the uterine septum, some women may show symptoms like:

  • Recurrent pregnancy loss
  • Infertility
  • Repeated IVF implantation failure
  • Pelvic pain (though rare)
  • Dyspareunia (painful sexual intercourse) in certain cases
  • Abnormal fetal placement or preterm birth during pregnancy

It is important to note that these septate uterus symptoms may also occur due to some other gynaecological conditions. If you face recurrent pregnancy loss, infertility or prolonged pelvic discomfort, consult a fertility specialist for a detailed evaluation and accurate diagnosis.

Causes of a Septate Uterus

A septate uterus is a congenital condition that means it develops before birth and is not caused by lifestyle, exercise or any activity during adulthood. The condition happens when the uterus does not develop properly during fetal growth.

Congenital Development of the Uterus

During fetal development, the uterus forms from two Müllerian ducts. Normally, these ducts fuse and the tissue between them disappears to create a single uterine cavity. When this tissue does not completely disappear, it forms a uterine septum that partially or completely divides the uterus.

Is a Septate Uterus Genetic?

The septate uterus is a congenital condition that is present from birth and is not believed to have any genetic links, though the exact cause is still not fully understood. The family history is usually not linked with the development of a septate uterus.

Diagnosis of Septate Uterus

A septate uterus is normally diagnosed during a fertility assessment in case of recurrent miscarriages or during a routine pelvic examination. Since it can resemble other congenital uterine abnormalities, accurate diagnosis is important before planning septate uterus treatment. Your doctor may recommend one or more of the following tests.

Medical History and Physical Examination

The doctor will assess your menstrual history, reports of previous pregnancies, miscarriages, infertility problems, and septate uterus symptoms. While a physical exam alone cannot diagnose a uterine septum, it is still valuable in guiding further assessment and ruling out other possible causes.

3D Ultrasound

Three-dimensional transvaginal ultrasound is typically the primary and highly precise imaging test used to diagnose a septate uterus. This imaging test creates images that clearly differentiate the uterine septum from other uterine problems.

MRI Scan

In case if the USG findings are not conclusive or additional information is required, an MRI scan may be advised. It helps assess the size and extent of the uterine septum and supports treatment planning.

Hysterosalpingography (HSG)

An HSG uses a contrast dye and X-ray to evaluate the uterine cavity and check whether the fallopian tubes are open. It can identify abnormalities within the uterus and is commonly performed as part of an infertility evaluation.

Diagnostic Hysteroscopy

Diagnostic hysteroscopy uses a thin telescope inserted through the cervix that allows the doctor to directly examine the inside of the uterus. It confirms the presence of a uterine septum and helps determine whether hysteroscopic treatment is required.

Treatment Options for Septate Uterus

The right septate uterus treatment depends on the size of the septum, symptoms, reproductive history, and whether you are planning a pregnancy.

When Is Treatment Required?

Not every septate uterus requires treatment. Your doctor may recommend surgery if you have:

  • Recurrent miscarriages
  • Difficulty conceiving
  • Repeated IVF implantation failure
  • Pregnancy complications linked to a uterine septum

If the septum is small, you have no septate uterus symptoms, and have had normal pregnancies, treatment may not be necessary. Your doctor will recommend the best approach based on your medical history and imaging findings.

Hysteroscopic Septum Resection

Hysteroscopic septum resection is a minimally invasive and commonly advised procedure for the treatment of a septate uterus. Since it is performed through the vagina, there are no abdominal cuts or visible scars.

The surgeon uses a hysteroscope to carefully remove the uterine septum in order to restore the normal shape of the uterine cavity. The procedure does not take much time and usually completes within 30 to 60 minutes, where most women go home the same day. Recovery is also normally quick and most patients resume their daily activities in a few days.

Fertility-Sparing Treatment

Hysteroscopic surgery preserves the uterus and supports future fertility. By removing the septum, it creates a healthier uterine cavity for embryo implantation and pregnancy.

Women who have experienced recurrent miscarriage or infertility due to a septate uterus often see improved pregnancy outcomes after treatment. If other fertility issues are present, your doctor may recommend IVF or other fertility treatments alongside surgery.

Non-Surgical Management (When Surgery Is Not Needed)

In cases of a small and asymptomatic uterine septum, surgery may not be required. Regular follow-up and monitoring are usually advised and if pregnancy occurs, the doctor carefully monitors it and identifies any complications without delay. Surgical intervention is only advised in cases where the septum is likely to harm pregnancy.
