ICSI Treatment in India

A lot of questions can come to your mind when you think of starting your fertility journey, especially if you are unable to conceive despite trying for several months or even years. For some couples, difficulties with sperm quality, sperm count, or fertilisation may make natural conception challenging. In these cases, Intracytoplasmic Sperm Injection (ICSI) can be the answer. ICSI is one of the most advanced reproductive technologies used today. It is a specialised procedure that helps with conception when traditional techniques fail. At Birla Fertility & IVF, our experts utilise the latest technology along with the experience of our embryologists to help you move forward with your journey to parenthood.

What is ICSI Treatment?

Intracytoplasmic Sperm Injection (ICSI) is an advanced approach that involves the selection of one healthy sperm, which is then directly injected into a mature egg inside the laboratory.

Different from standard IVF treatment, when the sperm and the egg are left to fertilise naturally in the lab setup, ICSI assists the fertilisation process by facilitating penetration of the egg by the sperm. This treatment method greatly enhances the possibility of fertilisation if sperm related issues are interfering with conception. After fertilisation, the embryo is observed in the laboratory before being implanted into the uterus.

Who May Need ICSI Treatment?

ICSI may be recommended when fertilisation is unlikely to occur naturally or through conventional IVF alone. After evaluating both partners, your fertility specialist may suggest ICSI in the following situations:

  • Oligospermia (Low sperm count)
  • Poor sperm motility
  • Abnormal sperm morphology
  • Azoospermia (absence of sperm in semen)
  • Previous IVF cycle with poor or failed fertilisation
  • Use of surgically retrieved sperm
  • Frozen sperm samples with limited sperm numbers
  • Unexplained infertility after repeated treatment failures
  • Fertility concerns related to advanced paternal age
  • Certain genetic conditions affecting sperm quality
  • Couples planning preimplantation genetic testing (PGT)

ICSI is particularly beneficial for couples experiencing male-factor infertility, as it helps overcome many barriers that may prevent successful fertilisation.

Benefits of ICSI Treatment

ICSI has revolutionised fertility treatments, enabling many couples to achieve a successful pregnancy. Some of the advantages of ICSI include:

Higher Fertilisation Rates

One major benefit of ICSI is the increase in the possibility of fertilisation through the direct injection of a sperm into an egg. Through this process, it is possible to prevent many obstacles that could hinder the natural fertilisation process.

Effective for Severe Male Infertility

ICSI is most suited for cases where there are problems with male infertility, such as low sperm count, poor sperm motility, or abnormally shaped sperm. Even when sperm quality is significantly compromised, successful fertilisation may still be possible.

Useful with Surgically Retrieved Sperm

If men do not have any sperm in their semen, a condition known as azoospermia, because of blockages or any other health problems, they can still have biological children. ICSI can be performed using sperm obtained through specialised surgical retrieval procedures like Micro-TESE

Reduced Risk of Fertilisation Failure

Since fertilisation takes place in a laboratory environment, the chances of fertilisation of the mature eggs are greater. This is particularly useful for couples who have failed fertilisation in previous rounds of IVF treatment.

Supports Genetic Testing

ICSI is commonly used when embryos are planned for preimplantation genetic testing (PGT). Facilitating controlled fertilisation, it helps support accurate genetic assessment before embryo transfer.

Enables Use of Frozen Sperm

It is possible to conduct ICSI using frozen and thawed sperm, especially in cases where there is a limited availability of viable sperm. This makes it a valuable option for fertility preservation and specific medical situations.

ICSI Success Rate and Factors Influencing Success

While fertilisation rates with ICSI can often exceed 70-80% of mature eggs, the success rate for pregnancy and live births will differ from one couple to another. Some of the things that could determine the success rate of the process are as follows:

Female Age

The age of the female partner is one of the main factors that determines the success rate of ICSI. Younger females usually have higher-quality eggs and fewer chances of chromosomal abnormalities, which helps improve fertilisation and pregnancy rates.

Ovarian Reserve

Ovarian reserve is used to indicate how many eggs are left in a woman's ovaries. When a woman has a healthy ovarian reserve, enough mature eggs can be produced, which increases the chances of getting healthy embryos.

Sperm Quality

Despite the fact that ICSI can help in solving many sperm-related challenges, the health of the sperm remains an important factor that influences the result of the procedure. Factors such as sperm DNA integrity, motility, and morphology can influence fertilisation and embryo development.

Embryo Quality

The quality of the embryo is one of the most important factors that affect the outcome of the procedure. Normally developing embryos that reach the desired stage of growth tend to have better results.

Uterine Health

A healthy uterus and receptive endometrial lining are essential for embryo implantation. Conditions such as fibroids, polyps, adhesions, or untreated uterine abnormalities may affect the chances of pregnancy.

Lifestyle Factors

Some lifestyle factors can influence the fertility of couples and the success rates of their treatment. These include smoking, high alcohol consumption, obesity, improper diet, insufficient sleep, and high levels of stress that might adversely influence the qualities of eggs and sperm.

Laboratory Standards

The expertise of the embryology team and the quality of laboratory facilities become key to ICSI results. Modern laboratory equipment with high-quality standards, and skilled embryologists contribute to creating ideal conditions for successful fertilisation and further embryo growth.

Diagnostic Tests Before ICSI Treatment

Proper diagnosis forms the foundation of a fertility treatment. Before curating a personalised treatment plan for you, your fertility specialist may recommend some diagnostic tests to identify the root cause of the problem.

For Women

The following tests may be prescribed to the female partner:

For Men

The following tests may be prescribed to the male partner:

How to Prepare For ICSI Treatment?

A simple preparation before your treatment can support the outcomes. Your doctor may advise you to

  • Maintain a healthy weight
  • Eat a balanced fertility-friendly diet
  • Take prescribed supplements
  • Quit smoking and tobacco use
  • Limit alcohol consumption
  • Manage stress levels
  • Get adequate sleep
  • Exercise regularly

It is important for both partners to actively participate in pre-treatment preparation.

What Happens After ICSI Treatment?

Following embryo transfer, the waiting period can feel emotionally challenging.

During this phase:

  • Hormonal medications may continue
  • Mild cramping or spotting may occur
  • Daily activities can usually be resumed
  • Follow-up appointments may be scheduled
  • A pregnancy blood test confirms treatment outcome

If pregnancy is achieved, further scans will be planned to monitor fetal development.
