A 40-year-old couple came to us with secondary infertility after being unable to conceive again despite already having a 10-year-old child. The patient had severely diminished ovarian reserve with AMH of 0.1 ng/mL and AFC of 2, while advanced maternal age significantly increased the risk of chromosomally abnormal embryos. The Treatment Plan that made the Difference

IVF-ICSI with PGT-A Strategy

An IVF-ICSI self-cycle with PGT-A was planned to improve embryo selection and reduce unsuccessful transfers.

First IVF Cycle

Two mature oocytes were retrieved and fertilized through ICSI. Both embryos developed into blastocysts and underwent PGT-A testing. However, both blastocysts were reported as aneuploid.

Second IVF Cycle

In the second cycle, two mature oocytes were again retrieved and fertilized using ICSI. One high-quality 4AA blastocyst was formed and identified as euploid after PGT-A testing.

Frozen Embryo Transfer (FET)

A frozen embryo transfer was successfully performed using the euploid embryo.

A Story of Precision Embryo Selection in Advanced Maternal Age

This case highlights that euploid embryos can still be achieved even in severe poor ovarian reserve cases. It also demonstrates the importance of persistence through multiple IVF cycles, advanced embryology support, and PGT-A-based embryo selection in advanced maternal age patients. With individualized IVF planning and careful embryo selection, successful ongoing pregnancy outcomes are possible even with very limited embryo numbers.
