Ongoing Pregnancy After Three Failed IVF Cycles and Recurrent Implantation Failure
After nearly nine years of secondary infertility and three unsuccessful IVF cycles, this couple came to Birla Fertility & IVF, Bhubaneswar, looking for a fresh assessment of why repeated embryo transfers had not resulted in pregnancy.

Journey Timeline

1. Three Failed IVF Cycles

2. Sperm & Uterine Reassessment

3. Microfluidic Selection + ICSI

4. Personalised Frozen Embryo Transfer

5. Ongoing Clinical Pregnancy

Recurrent Implantation Failure
The couple had experienced three unsuccessful embryo transfers, including Day 3 and blastocyst-stage transfers, making it important to reassess potential factors affecting implantation.

Male-Factor Infertility
The male partner had teratozoospermia, which meant a high proportion of sperm showed abnormal morphology.

Long-Standing Secondary Infertility
After nearly nine years of trying to conceive again, the couple had already undergone multiple fertility treatments without achieving an ongoing pregnancy.

The Treatment Plan that made the Difference

Microfluidic Sperm Selection and ICSI
Microfluidic sperm selection was incorporated before ICSI to help select sperm for fertilisation in the setting of abnormal sperm morphology.

Extended Blastocyst Culture
Embryos were cultured to the blastocyst stage so their development could be assessed before cryopreservation and transfer.

Uterine Assessment and Preparation
Hysteroscopy and personalised endometrial preparation were performed before embryo transfer to assess and prepare the uterine environment.

Personalised Frozen Embryo Transfer
Two good-quality blastocysts, graded 3AA and 3AB, were selected for frozen embryo transfer. PRP support and a specialised embryo-transfer medium were also used as part of the individualised transfer strategy.

The Outcome
The frozen embryo transfer resulted in a positive pregnancy blood test, followed by confirmation of an ongoing clinical pregnancy. This case highlights the importance of reassessing both embryo-related and uterine factors when implantation has repeatedly failed.

The case reflects how repeated implantation failure may require a fresh, individualised assessment before planning another transfer.
